General Policies


Families and friends are encouraged to visit residents often. Visiting hours are from 9:00am to 9:00pm (encompassing the noon and evening mealtimes). Facility staff request that visiting times please be geared to their resident's daily routine, condition and needs. Visiting hours may be waived in the case of a seriously ill resident at the discretion of the Nurse in Charge. The Administration does request that families please use discretion by not visiting when they may have a contagious illness or are otherwise not well able to visit; they will thus be sparing their relative or other residents the concern of an additional health problem. Children of any age are welcome and encouraged to visit. The Administration and staff request that visitors please respect the rights and privacy of other residents. (For greater detail and explanation please refer to the "Rights and Responsibilities of Visitors" provided to residents and families or responsible parties at the time of admission).

Off-Premises Visitation

Residents, according to their physical condition, may leave the nursing home premises with family, or friends for family gatherings, shopping, car rides, etc. with the prior approval of the attending physician. Hence, families should contact the nursing facility and/or the attending physician a day or two prior to a planned off-premises visit. The signing of a release and information form is required before the resident may leave the grounds.

With prior arrangements and at the discretion of the Charge Nurse at the time of departure, residents may also leave the facility for overnight or several days. Residents should consult their sponsoring agency and/or insurance agent regarding leave-of-absence regulations.

The Nurse in Charge may provide medications to the resident or family for use during off-premises visiting or leave of absences overnight or for several days. Therefore, families or responsible parties should make arrangements with the Nurse in Charge for any medications continuity of care while the resident is away or the attending physician immediately if there are any problems or questions.

Family or Responsible Party Vacations and Changes in Address
It is expected that all changes in family addresses and telephone numbers be registered with the Administrative Secretary, Social Services worker or the Nurse in Charge. It is most important that families notify the facility when they will be vacationing or out of town. If possible, families should leave a number where they can be reached in an emergency or the name and telephone number of an alternate relative or responsible party.


The skilled nursing facility will launder underclothing, nightgowns, pajamas and other machine - washable articles without charge, but will assume no responsibility for the intactness of these articles. Families or other responsible parties are requested to attend to the articles of clothing requiring hand laundering or dry cleaning. If they prefer to launder all personal clothing, then family may make such arrangements with the Nurse in Charge. It is expected that residents and their families will plan the replacement of clothing and other personal articles with the nursing staff.

Clothing will be labeled by the facility with his or her full name either by sewn-on or iron-on name tape or laundry pen. Washable clothing is requested. It is expected that family or other responsible party will have sufficient numbers of articles which are comfortable, modest and appropriate to the resident's condition, needs and preferences.   • Appropriate outerwear by season and as warranted by the resident's needs.

Any articles of clothing brought to the facility after admission, i.e. gifts, replacement items, etc. should be taken to the nursing station in a bag with the person's name so that they can be appropriately marked before being placed in the room.

Toiletries and Other Personal Articles

Articles used in regular oral hygiene and skin care are provided by the facility. If a resident prefers a different brand or type of item than that provided by the facility or if the attending physician recommends a medicated or other special type of item, then it is the responsibility of the resident or responsible party to provide such an item. Cosmetic items are not provided by the facility.

Eyeglasses, Dentures, Hearing Aids and Other Appliances

The staff of the facility take every reasonable precaution to safeguard eyeglasses, dentures, and hearing aids but cannot be responsible for their breakage or loss. Families may want to leave extra glasses, batteries for hearing aids, etc. with the Nurse in Charge. When visiting, families or responsible parties should check periodically to ascertain whether or not replacement articles are needed. The obtaining, repair, and re-examination for such articles are the responsibility of the resident, family or responsible parties.

Patient Incidental Account, Money and Jewelry

Prudence dictates that residents not have cash in their rooms or on their persons. A Patient Incidental Account of either interest-bearing or regular type are available for residents and responsible parties who wish to leave money at the facility. Such an account may be opened with the Receptionist and must maintain a minimum balance of twenty dollars. A ledger is kept of each resident's account transactions and a quarterly accounting is provided to each resident and/or responsible party or sponsoring agent. Residents and their families are encouraged to avail themselves of this service from the day of admission since experience has shown that incidental needs do arise. If families wish to leave money for the account during weekend visits, please see the Nurse in Charge. We advise, however, that such business be conducted during regular business hours or by mail in consideration of the nursing staff's primary care responsibilities.

The Sunset Nursing and Rehabilitation Center cannot be responsible for money or other valuable items. We suggest that residents not bring jewelry or other valuable keepsakes. If, however, they must be brought to the facility, then they may be placed in Valuables Envelopes in the Social Service Office. Residents are welcome to wear wedding rings and watches unless prohibited by their physical condition. Families are requested to advise the Nurse in Charge when removing any belongings of the resident from the facility.

Building Access and Alarm System

Among our efforts to safeguard our residents is a door alarm system, which can help alert our staff if a resident leaves the building unattended. In this way our ambulatory but confused resident can walk about the entire building ad lib but have additional protection from the hazards of leaving the building unattended. Although the entire building has the door alarm system, a by-pass mechanism has been installed at the two main entrances (A wing and between B & C wings) so that visitors, staff and attended residents can exit the building via those entrances without triggering the alarm. We ask that you please use only those entrances.

On the wall near each exit you will find a panel of numbers aligned as they appear on touch-tone telephones. Simply press the numbers of the Boonville zip code: 13309. The red indicator will go off and the green light will come on. The alarm is by passed for sufficient time to egress at normal pace.

Upon entering the building simply press the black button near the doorway. As before, please do not assist a resident from the building unless the nursing staff on their wing is aware of their departure from the building.

Personal Televisions

Individually owned televisions sets are permitted in a patient's room subject to inspection and approval by the Maintenance Department. Maintenance of a resident's set is sole responsibility of owner. Table models must be placed on appropriate stand. They cannot be placed on over the bed tables or bedside stands. Console sets must be of reasonable size. No metal cabinets are permitted.

Mail & E-Mail

Resident's mail is delivered to the Nursing Home daily via the United States Postal Service. Letters and packages should be addressed as follows:

Admission Agreement

Upon acceptance for admission to the Home, the resident or responsible party or sponsor signs an Admission Agreement at the time of the actual admission.

The admission Agreement specifies charges, services and refund policy. On the day of admission, self pay resident are required to make an advance payment covering 30 days of Routine Service.

Patient's Rights

In accordance with State and Federal Regulations the Home has adopted policies on Patient's Rights. A copy of the 'Patients Bill of Rights' will be provided at the time of admission and can be viewed by clicking here.

Special Diets and Bringing in Treats

Food or candy in reasonable amounts and in appropriate airtight containers may be brought to the resident. However, visitors should check with the resident or the nurse in charge regarding any special diets and the appropriateness of the type of food item to the resident's condition. An item which may be a treat for one person may be injurious to the health status of another. The best treat you can bring is yourself for a pleasant, positive visit.


Residents may smoke only in the designated smoking lounge on B-Wing. Residents must be accompanies by a staff member at all times. Visitors are permitted only to smoke outside of the building. Residents are to leave all matches, cigarettes, lighters, cigars, pipes and tobacco at the nurses' station.


It is neither customary nor acceptable that residents or their families engage in tipping personnel. Individual staff members are not allowed to accept gifts so please do not make it difficult for them to decline. If you wish to express appreciation, smiles and kind words are most welcome.

Concerns and Suggestions

In order to maintain and improve the level of quality care, we invite your input. Please share any concerns or suggestions with the nurse in charge, social service personnel or the administrator. As an integral part of the health care team at Sunset we need your input.

Room Transfer

During the course of a resident's stay it may be advisable or necessary for them to transfer from one room to another. Room changes are kept to a minimum in order to minimize the potential effects of adaptation to transfer change. The facility does attempt to lodge residents in such a way that there will be a congenial and comfortable room situation for each resident as the situation and resident population characteristics allow. If two residents are married, they are permitted to share a room if they wish, unless medically contraindicated. Residents are advised that the Medicare, Medicaid and other health insurance programs provide payment for a semi-private room only. A room change may be necessitated by certain conditions or treatment plans (i.e. isolation precautions). A decision regarding room change is made by the Director of Nursing, Administrator and Social Services in consultation with and with the approval of the resident(s) as his or her condition allows.

Facility Transfer

Residents may require transfer to another facility for temporary acute hospital care. The attending or alternate physician will make this decision and will arrange for admission to the hospital taking into consideration, if possible, the preference of the resident. Hospital and transportation changes are the financial responsibility of the resident, responsible party and/or sponsoring agency or insurance.

Bed Reservation Policy

Sunset Nursing and Rehabilitation Center will reserve the bed of any resident during hospital stay or leave of absence for the length of time stipulated by the person or agency that pays for their stay. Full payment for each day at the established rate is expected. Clothing, personal items and other belongings of the resident may remain in their room or in safekeeping. It is particularly important not to leave money or other valuables unattended in the room during a period of hospital stay or leave of absence. Such article should be taken home or placed in safekeeping in the office. By prior arrangement with the physician or pharmacist, medications may be taken with the resident on leave of absence for the pre-stated length of leave.

Upon re-admission from hospital stay or leave of absence residents agree to resume adherence to the conditions of their stays as outlined at the time of original admission.

It is vital that resident be aware of the specific bed reservation policy of their sponsoring agency, if any. Private pay residents are encouraged to make their wishes known to the person who helps them attend to their affairs.

Veterans Bed Reservation Policy

The Veterans Administration Contract Placement Program through the Syracuse VA Medical Center offers reservation of the bed of a veteran for whom they are paying for their skilled nursing care for a period of forty-eight hours. Sunset Nursing and Rehabilitation Center is required to notify VAMC of the hospitalization of any contact placement resident within forty-eight hours of hospital transfer. Records are kept for VA review upon their request of all leaves of absence of contract placement residents.

Medicaid Bed Reservation Policy

Residents whose payment for skilled nursing care is by the Medical Assistance Program of the New York State Department of Social Services (Medicaid) are entitled to bed reservation if they have been in residence at the facility for a least thirty (30) days. Under their policy a bed is reserved for a resident for a period up to twenty days. Residents should be aware that the Medicaid bed reservation policy applies to active cases and may not be applicable to pending cases by the Department of Social Services. In the event the resident's hospitalization or therapeutic leave extends the bed hold period, the resident shall be readmitted to the facility immediately upon the first availability of a bed in a semi-private room. If the resident requires the services provided by the facility and is eligible for Medicaid nursing home services. If the resident has been in residence at least 30 days, as soon as the first bed becomes available in a semi-private room if resident was hospitalized, transferred or discharged on therapeutic leave without being given a bed hold when the resident requires the services provided by the facility and is eligible for Medicaid. (If this bed happens to be in a room with a person of the opposite sex and you prefer accommodation with resident of the same sex, Sunset will advocate this right for you.)

Medicare - Covered Residents

The Medicare Program does not provide for reservation of a beneficiary's bed in a skilled nursing facility during a period of hospitalization. If a Medicare beneficiary wishes to reserve their bed during a period of hospitalization they must make arrangements privately or abide by the policy of their sponsoring agency.

Other Source of Payment or Reimbursement

Residents and/or their responsible parties should review their policy, consult their agent or discuss bed reservation potential with their sponsor or responsible party.

This policy is provided to residents and/or their responsible party at the time of admission and upon transfer. Hospitals are advised of the bed reservation status of a resident upon transfer.
